Before you start

A few honest notes

Please only share what's needed for us to respond well. Don't send detailed medical records or clinical notes through this form, a short summary of relevant support or safeguarding considerations is enough, and we'll pick up anything else by phone.

Being referred does not automatically mean a service is free. Some community activities are free, some opportunities are grant-supported, and some one-to-one services are privately paid, private mentoring is currently £90 per session. We'll always be clear about cost before anything starts.

Referrals are stored securely and seen only by the Wild Mind team. This form is not for emergencies, if someone is at immediate risk, contact 999 or their local crisis service.
