How to Make a Referral

Wild Minds accepts referrals from schools, local authorities, and commissioning teams for young people who require specialist, part-time Alternative Provision or post-16 support.

All referrals are subject to a suitability and capacity assessment to ensure that Wild Minds is the right setting to meet a young person’s needs safely and effectively.

Step 1: Complete the Referral Form below

Step 2: Send us a copy of the young person’s most recent EHCP
📧 Email to: office@wildmindwellbeing.co.uk or use this upload link

Step 3: Complete the Young Person's Risk Assessment Form
👉 Download the Risk Assessment Template

Step 4: Book a Visit to the Farm

Once we’ve received the required documents, we’ll carry out a suitability assessment and get in touch with the next steps.
